Responsibilities- Conduct comprehensive assessments.
- Create personalized treatment plans.
- Deliver high‑quality care to individuals in the comfort of their homes.
- Work closely with a diverse team of healthcare professionals to ensure seamless coordination of care.
- Monitor patient progress, adjust treatment plans as needed, and provide ongoing support to help patients achieve their goals.
- Flexible scheduling: work weekdays, weekends, or a combination to accommodate your lifestyle.
- A degree in Physical Therapy from an accredited university/program.
- A current, unrestricted state license as a Physical Therapist in the state of practice.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.
- Strong organizational and communication skills.
- Experience in Home Health Therapy and completion of OASIS documentation is preferred.
