Licensed Practical Nurse All Shifts

Shifts Available: Day, Evening, Night

Garden Spring Nursing & Rehabilitation Center is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our clinical team. We are hiring for full-time, part-time, and per diem positions across all shifts. The LPN will provide direct resident care, support clinical operations, and work collaboratively with the interdisciplinary team to ensure high-quality outcomes in a skilled nursing and rehabilitation environment.

• Provide direct nursing care in accordance with physician orders and individualized care plans
• Administer medications and treatments safely and accurately
• Monitor residents’ health status and report changes to the RN/physician
• Document care and services in the electronic medical record
• Assist with admissions, discharges, and transfers
• Supervise CNAs and support daily resident care activities
• Maintain compliance with infection control, safety, and regulatory standards
• Communicate effectively with residents, families, and healthcare team members

• Active Licensed Practical Nurse (LPN) license in the applicable state
• Graduate of an accredited practical nursing program
• Experience in long-term care, rehabilitation, or skilled nursing preferred
• Strong clinical, communication, and organizational skills
• Ability to work independently and as part of a team
• Commitment to resident-centered care
