Garden Spring Nursing and Rehabilitation Center is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our skilled nursing and rehabilitation team on Full-Time and Part-Time Shift Night shift (11PM-7AM). The LPN will provide direct resident care while working collaboratively with RNs, physicians, and interdisciplinary staff to ensure the highest quality of care.
Essential Duties & Responsibilities- Provide direct nursing care in accordance with resident care plans and physician orders
- Administer medications and treatments
- Monitor and document resident conditions and report changes
- Assist with admissions, discharges, and care coordination
- Ensure compliance with state regulations and facility policies
- Promote a safe, respectful, and resident-centered environment
- Current Pennsylvania LPN license in good standing
- Long-term care or rehabilitation experience preferred (new grads welcome)
- Strong communication, clinical, and documentation skills
- Ability to work effectively as part of a healthcare team
- Full-time and Part time, Night 11PM-7AM
