Garden Spring Nursing and Rehabilitation Center is seeking a dedicated and detail-oriented MDS Coordinator to oversee the completion and management of Minimum Data Set (MDS) assessments. The MDS Coordinator plays a vital role in ensuring regulatory compliance, accurate clinical documentation, quality resident care, and appropriate Medicare/Medicaid reimbursement.
Essential Duties & Responsibilities- Coordinate, complete, and submit MDS 3.0 assessments accurately and on time in accordance with CMS regulations
- Ensure compliance with Medicare, Medicaid, PDPM (Patient-Driven Payment Model), and state/federal guidelines
- Collaborate with the interdisciplinary care team (nursing, therapy, social services, dietary) to gather assessment data
- Develop, update, and maintain resident care plans based on assessment findings
- Track and manage assessment schedules, deadlines, and compliance calendars
- Audit medical records for accuracy, completeness, and regulatory adherence
- Educate staff on MDS processes, documentation standards, and PDPM requirements
- Participate in quality assurance (QA), audits, and survey readiness
- Monitor reimbursement indicators and identify opportunities for improvement
- Stay current with changes in CMS, MDS, and PDPM regulations
- Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of employment
- Minimum 1–3 years of experience in long-term care or skilled nursing
- Demonstrated knowledge of MDS 3.0 and PDPM
- Strong understanding of Medicare and Medicaid reimbursement regulations
- Experience with Electronic Health Records (EHR) and MDS software
- Excellent organizational, communication, and analytical skills
- Ability to work independently and manage multiple deadlines
- Current RN (unrestricted and in good standing)
- (Not always required, but highly desirable)
- RAC-CT (Resident Assessment Coordinator – Certified)
- RAC-CTA (Advanced)
- AAPACN MDS Certification
- Other MDS or care coordination certifications
- Preferred Experience
- Previous MDS Coordinator or Assistant MDS experience
- Experience with state and federal surveys
- Knowledge of quality measures (QMs) and Five-Star ratings
- Staff training or leadership experience
- Ability to sit, stand, and move throughout the facility
- Ability to review medical records and work on a computer for extended periods
- Office and clinical environment
