Hospice RN Case Manager; ALLENTOWN, PA

Job Overview

Guardian Hospice is revolutionary healthcare model, seeking a compassionate and dedicated Hospice Nurse to join our team. The ideal candidate will provide end-of-life care and support to terminally ill patients and their families. This role requires a high level of empathy, excellent communication skills, and the ability to provide comfort during challenging times.

• Previous experience in nursing, preferably in hospice or palliative care
• Ability to lift and move patients with care and compassion
• Familiarity with medical-surgical procedures
• Knowledge of hospice regulations and practices
• Experience working with individuals with developmental disabilities is a plus

• Conduct physical examinations and assessments of patients
• Administer medications and treatments as prescribed
• Collaborate with interdisciplinary teams to develop individualized care plans
• Provide emotional support and guidance to patients and their families
• Monitor patient symptoms and condition closely
• Educate patients and families on end-of-life care options
• Maintain accurate medical records

• Knowledge of the principles of the intake, admission, and transition to care process with the ability to further develop skills in these competency areas
• Ability to communicate effectively with patients and families, physicians, referral sources and colleagues
• Reliable means of transportation
• Currently licensed as an R.N. in the state of Pennsylvania
• Previous Hospice/Palliative care experience
• Strong Presentation

• The Admissions RN is a registered nurse and an Interdisciplinary Team member whose primary responsibility is to present Guardian's services and philosophy to patients and families in a manner that results in the admission of appropriate patients
• Participation in the information gathering, analysis, education and form completion aspects of the admissions process.
• Evaluate patients for hospice appropriateness, e.g. medical records review as requested.
• Obtain agreement for admission of referred patients and families who meet admission criteria.
• Analyze data collected to facilitate/obtain admission orders.
• Maintain clinical and professional competence.
• Take the initiative in developing effective relationships internally with the appropriate admissions and patient care resource persons.
• Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient s family has concerning hospice and palliative care.
