Licensed Practical Nurse (LPN) - PRN-A - Patriots Colony Flexible Hours

Position posted by Riverside Health in Williamsburg, VA. The role is a part‑time PRN (as needed) licensed practical nurse with flexible hours, requiring 4 shifts in a 4‑week period, 2 of which must be weekends.

Works under the supervision of a Registered Nurse, Nurse Manager or Nursing Supervisor, and ultimately reports to the Director of Nursing or Assistant Director of Nursing and Director of Quality Care Management. The LPN participates in the assessment, planning, implementation and evaluation of nursing care and provides for the emotional, physical comfort and safety of the residents and their families, while also assisting in the supervision of auxiliary personnel.

• Medications and treatments are administered accurately and according to provider’s orders.
• Allows resident choices within limits of capability considering safety/dignity issues and follows policies and procedures of facility to include resident privacy and dignity.
• Assigns duties to non‐professional staff such as CNAs according to education, training, policy and resident need in order to assure quality resident care is delivered. Engages team members for optimal performance and addresses performance as needed.
• Contributes to the development, maintenance, or modifications to the individual service plans. Identifies changes in condition with proper documentation according to nursing practice standards. Demonstrates consistent follow‑through for positive resident outcomes. Utilizes good judgment and specialized skills.

• 6 months LTC/AL LPN experience or 1 year LPN nursing.

• Licensed Practical Nurse (LPN) – Virginia Department of Health Professions (VDHP) upon hire (required).
• CPR/BLS Certification – American Heart Association, American Red Cross or American Safety and Health Institute within 30 days (required).

• Weekend shifts.
