Physical Therapist- Home Health

BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a full‑time or part‑time position performing home‑health visits. The office services adult and geriatric clients on a per‑visit basis in the Williamsburg to Yorktown, VA area.

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as range of motion, transfer status, stability, gait pattern, muscle strength and endurance, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence‑based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapeutic վտանգ, gait training, balance, and other interventions.
• Continuously assess and revise the PT care plan, and collaborate with nursing in the multidisciplinary care plan as appropriate.
• Educate and instruct clients, family members, or other representatives in rehabilitative care and activities that promote independence.
• Accurately document observations, interventions, and evaluations using a state‑of‑the‑art touch‑pad tablet.

• A current VA Physical Therapist‑PT license.
• At least one year of recent work experience as a PT.
• Graduation from an APTA‑approved program or an AMA body‑approved program, documented by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving skills.
• Solid computer skills; prior EMR experience preferred.
