General Summary
Under the supervision of the Director of Nursing of the Transitional Care Unit, the Licensed Practical Nurse administers direct nursing care to patients and performs a variety of indirect patient care duties. The Licensed Practical Nurse is responsible for all professional working policies pertaining to this section.
Brief DescriptionThe Licensed Practical Nurse must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her unit – adolescent through geriatric. The Licensed Practical Nurse must demonstrate knowledge of the principles of growth and development over the life span; possess the ability to assess data reflective of the patient's status;
interpret the appropriate information needed to identify each patient's requirements relative to their age‑specific needs; and provide the care needed as described in the Nursing Department's policies and procedures. Type of supervision exercised: none. Jobs reporting to this position: none.
- Graduate of an accredited Licensed Practical Nurse Program required.
- Minimum 2 years experience in a long term care setting preferred.
- Possession of a current license as a Practical Nurse issued by the Pennsylvania State Board of Nurse Examiners required.
- Must maintain the standards of nursing care and implement the policies and procedures of the Nursing Department and Allied Services.
- Must be able to demonstrate the knowledge and skills necessary to provide individualized care appropriate to the age specific need and culture of the patient served.
Medium work: duties require exerting 25 to 50 lbs. of force occasionally (approximately 10‑33% of shift), and/or 10‑25 lbs. of force frequently (approximately 33‑66% of the time), and/or a greater than negligible amount up to 10 lbs. of force constantly to lift, carry, push, pull, and move objects. Position involves bending, reaching, stooping, and lifting (occasionally in a strained position) throughout the shift.
Ability to transport self throughout the facility is required.
- Occasional exposure to blood and body substances.
- Exposure to needle sticks possible.
Works in a well‑lit nursing unit, which includes the nurses station, patient’s rooms, treatment areas, etc.
