Overview
To work as part of a team to plan, review and provide support to young people 16-25, both preparing to leave care and supporting into adulthood having left care. To have responsibility for a caseload of young people and ensuring their needs are met. To facilitate safety and stability for young people while helping them achieve independence at their own pace.
You will work with partners, for example, in Housing, Health and education training and employment to provide a high-quality service to our Care Leavers and work to meet their needs holistically.
- Have responsibility for a caseload of young people and ensure their needs are met.
- Plan, review and provide support to young people 16-25, both preparing to leave care and supporting into adulthood.
- Facilitate safety and stability for young people and help them achieve independence at their own pace.
- Collaborate with partners in Housing, Health, Education, Training and Employment to deliver a high-quality, holistic service for Care Leavers.
- Understanding of the challenges facing Care Leavers and how to mitigate adverse life experiences.
- Passion for improving outcomes for care leavers.
- Ability to work in a challenging and fast-paced environment.
- Confidence in report writing and ability to advocate on behalf of young people where necessary.
- Desire to work with young people and support a successful transition into adulthood.

We are based in The Care Leavers Hub, which offers direct support to young people and facilitates a multi-agency working environment.
