Healthcare Nursing, Clinical Nurse Specialist
We are looking for an experienced and proactive Practice Nurse to join our established practice team on a 6 month fixed term contract. .
We would be looking for you to work between 15 - 20 hours possibly over 2 days.
The successful candidate is motivated and committed to providing high quality care, working collaboratively with the general practice team to meet the needs of our patients.

Main duties of the jobYour duties will include:
Running nurse-led clinics
Chronic disease management (e.g. asthma, COPD, diabetes, health promotion and lifestyle advice,Cervical screening, Immunisations and vaccinations, Contraceptive services - Depo, Supporting QOF and Primary Care Standards priorities, Maintaining accurate clinical records, Infection prevention and control

About usLocated in the heart of Wigan town centre, The Dicconson Group Practice is a well-established GP Surgery serving a diverse patient population of approximately 9650 people.
You will be joining a welcoming and experienced clinical team at the Practice.
This is headed up by 4 supportive GP Partners. We have a dedicated team of 3 salaried GPs, 2 Advanced Clinicians, 3 Practice Nurses, a Nurse Associate and our Healthcare Associate.
They are supported by our fantastic non-clinical team.,including our friendly reception team, 2 dedicated secretaries and Data Quality Lead as well as our management Team of 3.

Job responsibilitiesThe main duties of your job will include.
Running nurse-led clinics, Chronic disease management (e.g. asthma, COPD, diabetes, health promotion and lifestyle advice, Cervical screening, Immunisations and vaccinations, Contraceptive services - Depo, Supporting QOF and PCS priorities, Maintaining accurate clinical records, Infection prevention and control
You will work to Patient Group Directions, agreed protocols, within an evidence base and within definedcompetencies.
The Practice Nurse will assess patients and plan, implement and review treatment programmes.

Person Specification Qualifications- - Qualified RGN/RN/ NMC Registered
- - Applicants must have current UK professional registration
- - Experience of proactive working in a GP surgery setting
- -Proficiency in developing and implementing care plans for patients
- -Strong patient care skills with a focus on empathy and compassion
- - Good Interpersonal Skills
- - Good Team Player but also able to work autonomously
- - Computer literate with EMIS
- - Motivated, adaptable and enthusiastic
- Qualified in Phlebotomy
