Overview

Wigan Central Primary Care Network (PCN) is seeking to recruit an enthusiastic, motivated, and forward-thinking Clinical Pharmacist to join our expanding pharmacy team. This is a fantastic opportunity to become part of a well-established, dynamic, and supportive network, where pharmacists play an integral role in delivering high-quality, patient-centred care. You will join a collaborative team, working closely with GPs, nurses and other members of the multidisciplinary team to improve health outcomes for our local population.

In this role, you will be at the heart of medicines optimisation, promoting safe, evidence-based, and cost-effective prescribing across our practices. You will have the opportunity to develop your own clinical areas of interest, undertake Structured Medication Reviews (SMRs), and support patients with long-term conditions to achieve the best outcomes from their treatments. We are offering full-time working (37.5 hours per week).

Responsibilities
  • Provide expert clinical support to GPs and practice staff on all aspects of prescribing and medicines management.
  • Support safe and effective repeat prescribing processes.
  • Carry out medicines reconciliation following hospital discharge or transfer of care, ensuring safe and appropriate prescribing in line with local and national guidance.
  • Identify and proactively manage patients at high risk of medication-related problems, including those on high-risk or complex medicines.
  • Undertake Structured Medication Reviews (SMRs) and polypharmacy clinics, making independent prescribing decisions where appropriate.
  • Work collaboratively with community and hospital pharmacy teams to improve patient outcomes, reduce risk, and enhance continuity of care.
  • Provide advice and support to patients through clinics and telephone consultations regarding safe and effective medicines use.
  • Conduct clinical audits, contribute to quality improvement projects, and support achievement of Network targets and practice-based QOF indicators.
  • Assist in developing and implementing local prescribing guidelines, protocols, and pathways across the PCN.
  • Support public health initiatives and provide specialist input into relevant campaigns or health programmes.
