Community REACT are a well-established team, delivering care to individuals in their own place of residence. They may be acutely unwell, have fallen or in a crisis situation which puts them at risk of hospital admission if they don't receive immediate assessment and intervention. The team comprises of Physiotherapists, Occupational therapists, Nurses, ACPs, Doctors, support staff and a triage team. The main aim of the team is prevent unnecessary hospital admission.

We are seeking an Assistant Practitioner who has a keen interest in the field of admission avoidance and would like to develop their knowledge and skills in this area. Based at Claire House, you will work closely alongside and be supported by other specialist staff in the team.

You must meet the following criteria for this post:

  • As the role involves visiting patient homes & clinics across the Trust every day, postholders must state in their application if they have access to a car with suitable business insurance

This is a permanent full time role (37.5 hours) to be worked across Monday to Sunday.

An Enhanced DBS with Adults' Barred List check will be obtained during pre-employment checks.

Main duties of the job
  • Work as an integral part of the Interdisciplinary Team (IDT) that consists of Physiotherapists, Occupational Therapists, nurses, ACPs, Doctors and support staff to assist with the assessment and acute rehabilitation of patients in their own homes to prevent hospital admission or facilitate early discharge from A&E.
  • Promote and sustain independence/wellbeing for the individual or carer in the relevant community location.
  • Work autonomously within the clearly defined boundaries of the Learning development framework and carry out specific delegated clinical tasks and responsibilities, taking responsibility for their own workload.
  • Assess and provide equipment to patients at risk of admission to facilitate independence in a timely manner. Review relevant patients and the equipment that has been provided to ensure no additional problems arise.
  • Assist the wider team with supportive patients with activities of daily living prompting independence as able whilst overcoming acute illness or fracture.
Job responsibilities
  • Take responsibility for organisation and planning of a designated patient caseload, including basic evaluation and progression of treatments, reporting changes and responses to treatment in verbal and written format to the qualified therapists.
  • Conduct initial interviews to gather information, prioritise and analyse for the therapists.
  • Plan and prioritise own workload in accordance with service agreement.
  • Exercise good personal time management for completion of allocated workload.
  • Work in a flexible manner as directed and as service dictates.
Communications and Key Working Relationships
  • Develop rapport with the client before applying other technical/therapy skills where appropriate.
  • Communicate information in an understandable form to patients, carers and others.
  • Use effective communication requiring tact and persuasive skills to encourage patients to take an active role in their rehabilitation.
  • Communicate effectively with other members of the MDT.
  • Act as a liaison between therapist and others e.g. patients, families, carers.
  • Ensure records/documentation are maintained to required standard and report accurately when providing feedback to others.
  • Attend relevant care planning meetings, case conferences and MDT meetings as required.
Responsibility for Teaching
  • Maintain own competency to practice at Rehabilitation assistant level, through CPD activities and a portfolio reflecting personal development.
  • Demonstrate a commitment and evidence of Continuous Professional Development and to participate in departmental projects/audits.
  • Participate in the dissemination and sharing of information and skills with students and other staff, including supervision and training of new assistants.
