Main duties of the jobMain duties of the role will include (list not exhaustive):
To provide advanced nursing services to the patients of the practice
- Assess/examinepatients, diagnose, plan, treat and review/evaluate. Assessment may be face-to-face byappointment, by telephone as part of a triage/unscheduled care team andwill include patients presenting with an undifferentiated diagnosis andalso via patient on-line requests.
- Diagnose and manage patients presenting with both acute and chronic conditions based on amanagement plan which includes a medication plan and other interventions.
- Identify healthpriorities and make timely and appropriate interventions to support patients presenting in an emergency and/or with complex/urgent needs.
- Assess and manage patients physiological and psychological needs, including patients with complex needs, multiple diagnoses and polypharmacy.
- Identify patients atrisk of developing a chronic illness and develop a plan for minimisingrisk of worsening.
- Advise and educatepatients, promoting understanding, self-management, healthy lifestylechoices and sources of support and compliance with treatment plan andmedication.
- Ensure the healthneeds of all patients presenting to the nursing team are met in aclinically appropriate way and by the team member best qualified to meettheir needs.
- Ensure that all patients who may be vulnerable or who have mental health needs are appropriately managed and protected by the nursing team.
Our team currently consists of a Partner GP, 2 salaried GPs,1 sessional GP, 1 Practicenurse, 1 ANP, 1 HCA as well as a host ofadministration staff and clinical input from teams within our PCN, which include First contact Physio, Paediatric ANP and a Mental Health Practitioner.

Person Specification Qualifications- Specific advanced professional qualification which meets NMC requirements for high level practice - Advanced Nurse Practitioner qualification
- Experience of working in a Primary Care setting including GP Practice
- Experience with EMIS clinical system
- Experience in supporting and mentoring staff in training and development
- Able to provide advice and guidance to the nursing team
- Experience in risk assessments and identifying hazards
