To assist the Continence Advisors and Continence Nurse in the provision of continence care and support of patients and families in the community setting.

• Work as part of the Bladder and Bowel Care Service providing specific care, including psychological and emotional support to patients in the community setting, including nursing and residential homes and patient homes, as written in care pathway.
• Work autonomously.
• Undertake reviews of patients receiving continence supplies and assist monitoring the use of products.
• Responsible for ordering continence products or aids. Monitor stock control of equipment required.
• Work in accordance with Trust Policies and Procedures.
• Use a computer.

• Monitor skin integrity of patients at risk of pressure sores and provide preventative information.
• Provide Health Promotion advice e.g., monitoring diet and fluid intake, demonstration of products and aids.
• Document all information and report findings to the continence advisors/continence nurse.
• Provide clinical intervention as directed by continence advisors/continence nurse following training.
• Check equipment on a regular basis and ensure it is safe to use.
• Report Health and Safety or Infection Control issues.

• Develop own clinical knowledge and identify areas for development within Personal Development Plan.
• Participate in the development of others clinical practice and knowledge.
• Attend mandatory training in accordance with Trust Policies and Procedures.

• The ability to travel independently across the Trust. There will be a requirement to carry equipment with you.
• The ability to work flexibly in accordance with service needs.

• Demonstrable community experience.
• Ability to demonstrate basic continence knowledge through practice.
• Previous caring role.
• Training in aspects of continence care.

• NVQ in Health care level 3 or equivalent experience.
