Job Summary
Our client is seeking a dedicated Case Manager to join their team. The primary responsibilities for this role include assessing family safety, progress, and needs, conducting child safety check-ins, developing case plans, and providing coaching in various life skills. The ideal candidate will collaborate with community partners, maintain documentation, and attend court and training sessions.
Responsibilities- Meet with families in their homes weekly to assess safety, progress, and needs.
- Conduct monthly individual child safety check-ins.
- Complete assessments, develop case plans, and support families using evidence-based models.
- Provide hands-on coaching in parenting, communication, household safety, organization, problem-solving, and other life skills.
- Respond to family crises as needed (phone/text), including some evening or weekend support.
- Maintain accurate and timely documentation, case notes, and court reports.
- Collaborate regularly with internal supervisors, family mentors, and community partners.
- Appear in court and provide testimony as needed.
- Attend ongoing training and development sessions.
- Experience working with children, families, or vulnerable populations (employment or volunteer).
- Bachelor’s degree in Social Work, Human Services, Psychology, Education, or related field.
- Strong time management and organizational skills — able to balance documentation, visits, and crisis response.
- Comfort working in family homes, including environments that may be stressful or unpredictable.
Case Managers are required to work evenings regularly, typically 3 nights per week after 5 PM, driven by family availability, crisis response, safety checks, and home visits. On-call needs can occur during evenings, weekends, and crisis moments.

Eligibility for health benefits is based on verifying that an average of 30 hours per week during the first 4 weeks of the work assignment has been met. If you meet eligibility requirements and take action to enroll, you will be covered no earlier than 60 days into your assignment, depending on plan selection(s)).
- 401k (Eligible on the first 401k open enrollment date following 6 consecutive months on assignment. 401k Open Enrollment dates are 1/1, 4/1, 7/1, and 10/1).
