• Estimated Weekly Salary: $1,870–$1,970 per week

• License: Active Registered Nurse (RN) license in the appropriate U.S. state
• Experience: Home health RN experience required; field-based home health / homecare experience preferred
• Documentation: Proficiency with OASIS documentation required
• EMR: Experience with Homecare Homebase (HCHB) required
• Skills: Strong patient assessment, care planning, patient education, communication, and time-management skills in a field-based/home health setting

• Provide skilled nursing care and comprehensive clinical assessments to home health patients throughout the Metro Wichita service area
• Complete accurate OASIS documentation and maintain timely, compliant clinical records in Homecare Homebase (HCHB)
• Manage daily visit schedule to support productivity expectations (approximately 30 points per week)
• Develop and implement individualized care plans, patient education, and clinical interventions as part of the home health team
• Communicate patient status updates, care needs, and documentation requirements per assignment workflow and agency protocols
• Coordinate with interdisciplinary team members and support continuity of care for homebound patients
