RN - Case Management

Day Shift: Monday to Friday, 8:00 AM - 4:30 PM

The Case Management RN will provide comprehensive case management services, including assessments and discharge planning. The role requires a flexible, adaptable individual capable of thriving in a busy, fast-paced environment.

• Conduct case management assessments.
• Develop and implement discharge plans for patients.
• Collaborate with physicians and mid-level providers to ensure comprehensive care.

• Active RN license or Licensed Social Worker.
• Experience in HCA hospital settings preferred.
• Experience with Meditech and Midas systems is a must.
