Home Health Weekend LPN

Seeking an experienced Licensed Practical Nurse to join our Home Health team in the Wichita, KS and surrounding areas.

As a Phoenix Home Health LPN, you will be responsible for making skilled nursing visits by managing the healthcare needs of our clients in the comfort of their own home. The LPN should possess strong skills in physical assessment, diagnostic interpretation, and communication to complete the individualized plan of care according to physician orders that incorporates the analysis of initial assessment.

You will work alongside other nurses, Physical Therapists, Speech Therapists, and Occupational Therapists.

Pay Per Visit plus an additional weekend wage

Responsibilities

• Assist in the development, review, and revision of the plan of care.
• Provide nursing care in accordance with physician’s plan of treatment and individual plan of care, as authorized by the client’s payer source.
• Observe, record, and report reactions to treatment and any changes in the client’s condition to appropriate personnel and/or physician.
• Provide ongoing instruction and supervision of the client, family, or other health team personnel in the plan of treatment.
• Participate in education programs, quality assessment activities, nurse’s meetings, and other staff meetings as required.

Requirements

• Kansas or Compact Nursing License.
• Home Health experience required.
• Driver's License and Vehicle with Current Auto Insurance.
• Strong organizational and self-management skills.
• Excellent communication skills.
