RN Case Manager – Wesley Healthcare

We are seeking an RN Case Manager to join our senior case management team at Wesley Medical Center. The role focuses on providing comprehensive case management services for inpatient and observation patients, helping patients transition between care settings, and ensuring safe, cost-effective care coordination.

• Provide case management services for both inpatient and observation patients.
• Identify patients at risk for adverse outcomes during transitions of care.
• Conduct comprehensive psychosocial, medical, and discharge needs assessments for patients and families.
• Use evidence‑based tools and Inter Qual criteria to evaluate progression of care.
• Document professional recommendations, discharge plans, and coordination interventions for all health‑care team members.
• Act as liaison between physicians, patients, families, hospital staff, and outside agencies.
• Seek ways to control costs while maintaining patient safety and quality of care.
• Advocate for patient rights and needs, especially for under‑resourced populations.

• Current RN licensure in Kansas or valid multi‑state RN licensure.
• Basic Life Support (BLS) certification from the American Heart Association or American Red Cross.
• Minimum 3 years of clinical nursing experience in a hospital setting.
• Bachelor’s degree from an accredited institution preferred.
• Preferred experience:
  
  2+ years of case management or critical care, case management or utilization review certification, and Inter Qual familiarity.
