Targeted Case Manager

Position Summary

Responsible for assisting the individual with disabilities and their support network to identify, select, obtain and coordinate both paid services and natural supports to enhance the individual's independence, integration and productivity as outlined in the Person Centered Support Plan.

• Determine on an ongoing basis the individual's preferred lifestyle, strengths, resources and basic needs.
• Facilitate the development, update and monitoring of Person Centered Support Plans and Person Centered Service Plans with participation of the individual and their support network.
• Identify, secure and coordinate supports and services outlined in the plans.
• Maintain regular communication with other providers of services identified on the plans.
• Provide oversight to monitor the individual's satisfaction and quality of services.
• Advocate with the individual and their support network to gain access to needed services, entitlements and seek modification of service systems when necessary to increase accessibility and appropriateness.
• Participate in transition and portability plans for individuals leaving institutions, transitioning from high school to adult services, or other life transitions.
• Maintain documentation for tracking targeted case management, case notes, billing and program evaluation in compliance with KDADS, SCDDO, Medicaid/Kan Care and other regulatory guidelines.
• Maintain accurate and current documentation as required by the organization.
• Complete reports and compile information as needed by CPRF, SCDDO and/or KDADS.
• Meet requirements for Targeted Case Management as stated by SCDDO and/or KDADS.
• Report all suspected cases of abuse, neglect or exploitation as required by DCF/APS, SCDDO, KDADS and the agency.
• Treat persons with disabilities with respect and dignity.
• Protect confidentiality of information unless released by signed consent.
• Participate in training as required by KDADS and SCDDO to remain qualified to provide Targeted Case Management services.
• Submit billable activity and case notes in a timely manner, meeting the minimum monthly expectation of 70% of time billed.
• Transport clients to appointments or other service providers as necessary.
• Perform other duties as directed by Director.

Normal physical exertion. Requires sitting, standing, bending and reaching. Requires manual dexterity sufficient to operate computers, copiers, fax machines and telephone. Requires normal range of hearing and vision and must be able to meet clients, referral agencies and other resources in the community and county.

Daily mental concentration required; some creativity required for assessing methods and procedures. Must handle diverse work problems on a daily basis and demonstrate personal maturity and sensitivity.

Office and residential environments; surroundings may be dirty or noisy. 40‑45 hours per week are required for full‑time; travel daily to meet with clients, support network and community resources.
