Nurse; PACT - Outpatient

Overview

The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) executes position responsibilities that demonstrate leadership – experience – and creative approaches to management of complex primary care. This nurse provides gender‑specific direct Veteran care and care coordination – as it pertains – to Veterans and their care within the Primary Care Department. Manages assigned panel(s) – utilizes registries – and ensures appropriate evaluation and access is provided to patient panels.

• English Language Proficiency – required by 38 U.S.C. 7403(f)
• Graduate of an accredited professional nursing program (ACEN or CCNE)
• State license – full, active, unrestricted registration in a U.S. State, Territory or Commonwealth
• Master’s level bridge program participants may be appointed on a temporary basis while completing the program
• Foreign nursing education equivalency validated by a recognized equivalency evaluation (e.g., International Consultants of Delaware)
• Physical requirements – moderate lifting (15-44 lbs), moderate carrying (15-44 lbs), straight pulling, pushing, reaching above shoulder, use of fingers, both hands required, walking (4 hrs), standing (4 hrs), kneeling (0.5 hrs)

• Provide direct Veteran care, manage assigned panels, coordinate care focused on patient education and self‑management
• Administer medications and procedures per established policies and guidelines
• Collaborate with interdisciplinary team, influence care outcomes, implement evidence‑based decisions, evaluate outcomes at program or service level
• Document PACT outcomes, adhere to nursing process for direct patient care, assess‑plan‑implement‑evaluate care based on age‑specific components
• Support professional development of health care professionals, foster safe and supportive environment, provide constructive feedback for improvement
