HHSC is comprised of nine psychiatric hospitals, one youth residential treatment facility, and thirteen state supported living centers. - The Social Worker II performs and documents complex social services work including serving as a liaison between the individual, legally authorized representative, and others to assure ongoing care, treatment and support through the use of person-centered practices. This position gathers information to assess the individual support systems and service needs, including participation in interdisciplinary team meetings, as needed. This position completes an initial social history, as well as maintains appropriate updates.
This position may also support the assessment of the individual rights and capacity to make decisions and provides individual and group training to promote the exercise of rights. - Designs and provides individual and group training to promote the exercise of rights.
- Assists with the coordination of admissions, transfers, transitions, and discharges, as needed.
- Works under moderate supervision, with limited latitude for initiative and independent judgment.
- Performs other duties as assigned, including disaster response and/or COOP activation, which may require alternate shift patterns or locations.
- Attend work regularly and follow shift schedules, with possible rotating schedules, extended shifts, or overtime in accordance with agency leave policy. May serve in an on-call status as required.
- Interview and converse with individuals, legally authorized representatives, and involved persons to determine priorities and gather background information for assessing support needs in psychosocial, economic, and physical domains.
- Oversee communication and information exchange between individuals, representatives, involved persons, agencies, and treatment providers to ensure ongoing care, consent where required, and participation in interdisciplinary teams.
- Provide guardianship-related guidance and rights-based training in line with state and federal laws and policies.
- Oversee the coordination and completion of rights and capacity assessments and review documentation.
- Assign staff to admissions, transfers, transitions, discharges, and related activities to ensure access to services using person-centered practices.
- Design and conduct training to promote rights, voting, citizenship, and self-advocacy; facilitate group education and information gathering related to resident rights.
- Participate in residence and family meetings and conduct formal presentations describing facility processes.
