PIH Health Medication Management is a specialized subspecialty focused on comprehensive chronic disease management, working in collaboration with primary care providers and specialists across PIH Health Group and affiliated practices.
The Nurse Practitioner or Physician Assistant works independently to provide care for patients with acute and complex chronic conditions. This role includes performing and documenting comprehensive physical examinations, administering preventive care, conducting chronic illness surveillance, and assessing, diagnosing, and managing health-illness needs specific to conditions supported by the Medication Management clinic.

Two to five (2-5) years previous experience in chronic care outpatient setting
Required Experience- Current California Nurse Practitioner license
- Current Basic Life Support Certification
- Graduate from an accredited Nurse Practitioner
- Evidence of continuing education and self-development to support license requirements
- Board Certification:
Board certified by the National Certification Commission on Certification of Physician Assistants/Nurse Practitioners.
