Overview
The Case Management Coordinator plays a vital role in supporting the case management team by facilitating communication, coordinating care, and ensuring compliance with managed care requirements. This position involves outreach to members, assisting with care coordination, preparing case documentation, and supporting interdisciplinary team functions. The Case Management Coordinator will work closely with case managers, social workers, providers, and interdisciplinary teams to facilitate seamless care delivery.
The Case Management Coordinator assists with the management of members in High Risk Case Management, Complex Case Management (CCM), Special Needs Plan Case Management (SNP) and Coordination of Care including Transitions of Care (TOC).

- Facilitate communication within the case management team and with members and providers.
- Coordinate care and assist with care coordination documentation.
- Support interdisciplinary team functions and ensure compliance with managed care requirements.
- Conduct outreach to members and assist with transitions of care (TOC).
- Assist with the management of members in High Risk Case Management, Complex Case Management (CCM), and Special Needs Plan Case Management (SNP).
- Prepare and organize case documentation and records.
- Collaborate with case managers, social workers, providers, and other team members to deliver seamless care.
- High level of communication skills demonstrating courtesy and professionalism.
- Excellent organizational skills, problem solving and decision-making abilities.
- Strong attention to detail and ability to manage multiple tasks in a fast-paced environment.
- Proficient in Windows, Microsoft Word, and Excel.
- Minimum typing speed of 40 wpm
- Knowledge of medical terminology
- High school diploma or GED
- Minimum of two years of experience in health care field
- Experience in case management, managed care, or healthcare coordination
- Spanish Speaking
