OverviewProvides comprehensive case management, coordination, counseling, advocacy, linkage, and development of intervention plans using the START (Systemic, Therapeutic, Assessment, Resources, and Treatment) service model for individuals at risk for acute crisis, loss of residential placement, and/or individuals who are currently experiencing crisis. Ensures high quality services for participants and that all treatment provided follows protocols established and approved by ESSC. Supports both home and community based services and operations.
Responsibilities- Maintain an active caseload of individuals referred for START Services.
- Successfully complete the START Coordinator certification training within 12 months of employment.
- Use ecomaps for systemic analysis and provide consultation to systems/teams as needed.
- Assist with referrals for START services and screening for enrollment.
- Coordinate support meetings and crisis plan development for individuals enrolled in START.
- Conduct outreach visits to individuals’ home, day, and vocational settings regularly.
- Use the START Action Plan for case planning and strategizing.
- Develop Comprehensive Service Evaluations, Cross-Systems Crisis Prevention and Intervention Plans, intake/assessments, admission/discharge summaries.
- Develop materials and present cases at Clinical Education Team Meetings.
- Complete all required data entry expectations.
- Provide on-call support on a rotational basis as part of the START team.
- Participate in the Center for START Services National network, including online resources, conference calls, and annual conference.
- Master’s Degree in social work, psychology, counseling or human service field.
- Current CPR certification card.
- Complete START Coordinator certification within 12 months following employment.
- At least 2 years of professional experience working with individuals with developmental disabilities and behavioral health needs in a multidisciplinary team setting.
- Strong knowledge of evidence-based practices for individuals with developmental disabilities and behavioral health needs.
- Proficiency in Microsoft Office Applications.
- Strong clinical, administrative, and leadership skills.
- Excellent judgment, decision‑making, customer service orientation, and professionalism.
- Effective oral and written communication skills; ability to work cooperatively with various individuals and groups.
- Strong discretion, confidentiality, and ability to pass a post‑offer physical examination and TB test.
- Vaccination proof or acceptable declination.
- Clearance from DOJ and FBI; ability to obtain and maintain criminal record/fingerprint clearance.
- Local travel ability; compliance with transportation safety standards; valid driver’s license, insurance, registration.
- Pass all drug testing required by ESSC.
- Bilingual in Spanish (required).
