Complex Care Coordinator

As a care coordinator, you will be planning and rostering care staff into shifts and ensure that the care workers and nurses that are responsible for delivering care and support on a day-to-day basis attend their shifts as planned.

About the role
  • The care coordinator will organise rotas a month in advance for the complex care clients.
  • Building and developing strong relationships with staff members (carers and nurses)
  • Building and developing strong relationships with clients
  • Obtaining staff availability
  • Communicate with staff and clients in a respectful, professional, and friendly manner.
  • On call responsibility
About you
  • You will have strong communication skills both over the phone and via written communication.
  • You will be a problem solver, quick thinker and solution driven.
  • You will have worked in care and ideally will have coordination experience within either domiciliary or complex care.
  • You will have the right to work in the UK and a full UK driving licence.

  • Salary of £26,667 increasing after probation period to £30,000
  • Mon to Fri 9am – 5pm, plus on call to include some weekends and 1 evening / morning per week (1700 – 2300/0600 – 0900)
