Detailed Overview

Reporting to the Manager, Clinical Services and the Professional Practice chief for Social Work, you will provide psychosocial/emotional assessment and therapeutic clinical counselling services to clients (patients, residents, families, community clients) in the assigned clinical areas, using a variety of modalities and in accordance with professional standards and clinical policies and practices. You will participate on the interdisciplinary team in care planning, discharge planning and resource linkage from a Social Work perspective;

serve as a member of local committees and planning bodies as required; and provide education and consultation to clients and the interdisciplinary team regarding psychosocial/emotional aspects of illness, injury, disease, health, and Social Work practice.

Responsibilities
  • Conduct psychosocial/emotional assessments of clients by methods such as focused interviewing, collecting relevant data, interpreting data within the client system, using a variety of modalities and in accordance with professional standards and clinical policies and practices.
  • Provide basic therapeutic clinical counselling services to clients, based on a psychosocial assessment that identifies client/system strengths and challenges, offers interventions to improve psychosocial/emotional wellbeing, and establishes a plan of care including treatment goals, timelines and success indicators.
  • Educate and consult clients and healthcare professionals on Social Work practice and the assigned clinical specialty, providing information on high‑risk factors, recovery and community practice related to adaptation to illness and disability, trauma, bereavement, critical incident stress management, legislation, chemical dependency, and community living and support services.
  • Maintain client records by documenting assessments, charting client information, and preparing progress notes and reports in line with professional and organizational standards, policies, and procedures.
  • Offer input to the interdisciplinary team in client care planning and discharge planning by interviewing clients, assessing needs, articulating analysis and decisions, participating in team meetings focused on care and discharge, ensuring timely meetings, and collaborating with clients and community service partners.
  • Communicate with community health and social agencies—including government ministries, home health personnel and family physicians—by collaborating in assessment and management of at‑risk individuals, making referrals to appropriate resources when long‑term treatment or follow‑up is required, and exchanging relevant information.
  • Act as a resource to clients by providing information regarding community resources, facilitating access, suggesting contacts, arranging referrals, and developing community resources to support the health of families.
