Qualifications

To excel in this role, you'll need:

  • Licensure: A valid LGSW or LISW license in the state of Minnesota (Master's-Level Social Worker).
  • Experience: At least one year of clinical Social Work experience, with proficiency in Electronic Medical Records (EMR) preferred.
Responsibilities

As a valued member of our team, you'll:

  • Conduct home visits to assess patients' psychosocial status and needs.
  • Provide a range of Social Work services in the home, including short-term individual counseling, community resource planning, and crisis intervention.
  • Collaborate closely with the entire healthcare team to develop personalized treatment plans for each patient.
