Application Summary

WVUHS and the department of Emergency Medicine seek a full-time (0.9

FTE) Advanced Practice Professional (APP) to join the Wheeling team.

Duties
  • Manage comprehensive and individualized care for emergency medicine patients and their families in the DEM and immediate post discharge in the ambulatory setting. Coordinate hospitalization if necessary.
  • Complete pre-procedure evaluations.
  • Complete integrated assessments.
  • With the collaboration of emergency medicine faculty or by following established clinical Guidelines and Protocols, order additional tests or consults as deemed appropriate.
  • Perform and communicate results of diagnostic tests following established clinical guidelines and scope of practice.
  • Consult and communicate with DEM clinic and hospital staff regarding abnormal findings in the evaluations of patients.
  • Perform succinct comprehensive clinical evaluations and document findings in medical record.
  • Facilitate patient DEM discharge by coordinating appropriate services and tests prior to discharge necessary for outpatient follow-up appointments in the clinic (as applicable).
  • Develop a comprehensive individualized plan of care for emergency medicine patients, which includes teaching, discharge planning and necessary referral to other members of the health care team or admission as necessary.
  • Demonstrate expertise in caring for patients across the age continuum of their educational experience and certification.
  • Identify unstable or complex emergency medicine patient care situations and facilitate appropriate intervention in the DEM setting.
  • Recognize emergency situations and initiate effective emergency care.
  • Evaluate emergency medicine patients and serve as a coordinator for follow up procedures and tests in the ambulatory setting or upon admission (as applicable).
  • Assist in the collection of outcome measures (as applicable).
  • Demonstrate critical thinking and diagnostic reasoning skills in clinical decision making by the identification, evaluation and management of the individual needs of patients/families.
  • Demonstrate the ability to utilize equipment in a safe manner.
  • Base decisions made/actions taken on the relevant scientific principles, established standards of care and/or research findings.
  • Utilize teaching/learning theory and appropriate teaching strategies in the provision of education to nursing staff, hospital staff, students, patients, families and the community.
  • Promote an environment that facilitates learning by responding to appropriate patient/family preferences, priorities and readiness to learn.
  • Update own knowledge base throughout the year.
  • Complete all identified clinical competencies.
  • Serve as a resource person for staff caring for the emergency medicine patient.
  • Provide educational programs to professional and paraprofessional personnel in area of expertise.
  • Participate in community programs, distribution of literature, as appropriate in area of expertise.
  • Document the plan of care, interventions and evaluations in a clear and concise manner for every patient visit in the patient’s medical record following approved hospital policies and compliance standards for documentation.
