Description: This position provides custodial and personal care services to clients under the supervision of a Registered Nurse (RN) and in accordance with the Plan of Care.
Education: High School Diploma/GED required; current Nursing Assistant Certification in good standing with the State Board of Nursing.
Qualifications: Successful completion of formal training and certification as a Nursing Assistant per state law. At least one year of experience in home or long-term care preferred.
Reports to: Director of Nursing
Primary Duties:
- Maintain a clean, safe, and healthy environment in the client’s residence, including light cleaning and organization.
- Perform personal care activities as outlined in the Plan of Care, such as hygiene, ambulation, nutrition, and dressing assistance.
- Provide nutritional support and report any changes in the client’s condition.
- Assist with devices for daily living, range of motion exercises, and household skills.
- Monitor and record vital signs, fluid intake/output, and behavioral changes.
- Support self-administration of medication, including preparation, assistance, and record-keeping.
- Follow infection control policies and agency procedures.
Hours: Varies based on assignment; shifts available 24/7. Hours are not guaranteed.
FLSA Status: Non-exempt, hourly
