Job Description:

The Clinical Social Worker utilizes clinical expertise to perform psychosocial assessments, develop and implement plans of care appropriate care team, assess crisis situations to appropriately provide needed resources and or referrals for the patient, and to provide family and individual treatment as indicated. This position works collaboratively with patients, patients' significant other, families, healthcare providers, insurers, community resources, and all other involved parties.

Position Details:

Monday - Friday, 8:00 AM - 4:30 PM (in the field)
Essential Functions Assesses patients' medical and mental health history, along with their current presentation, to understand their treatment needs.

Organizes and prioritizes daily tasks by evaluating the needs of new, current, and discharging patients.

Diagnoses and treats various mental, emotional, and behavioral disorders, and performs social work consultations and psychosocial assessments.

Provides supportive, crisis, bereavement, and other necessary social work interventions, and develops care plans with input from patients, families, physicians, and the healthcare team.

Modifies care plans to reflect changes in patient status and refers patients to other services, such as community services, treatment programs, support groups, psychiatrists, or other mental health providers.

Educates, communicates, and coordinates with multiple patient care teams and outside agencies to ensure quality care, timely discharge, and resolution of delays and issues.

Acts as an advocate for patients and families, ensuring effective communication with the multidisciplinary care team, outside agencies, and other necessary resources.

Completes timely and accurate documentation in medical records, complies with clinical guidelines and billing standards, and adheres to federal, state, local, and clinical program standards, while working within the scope of the Mental Health Practice Act.

Skills Assessment Organization Diagnosis Consultation Intervention Coordination Advocacy Minimum  Qualifications Master's Degree in Social Work. Education must be obtained from an accredited institution. Degree will be verified.

Current State Licensure as LCSW or LMFT.Motor Vehicle Record Check.

Current driver’s license insured and reliable transportation and an acceptable driving record.

Computer skills (email, word processing, and spreadsheets).Excellent written and verbal communication skills.

Preferred Qualifications

Experience with patients, families, and treatment teams.

Physical Requirements Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.

Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.).May be expected to stand in a stationary position for an extended period of time.

For roles requiring driving:
Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. $35.25 - $54.39
