- Manages individualized, outcome oriented nursing care through the use of the nursing process.
- Provides care, treatment and services through the successful coordination and completion of a series of processes that include assessment, planning care, and providing care, in accordance with departmental and hospital policies and procedures.
- Medication Administration: demonstrates knowledge of and follows SSH policies and procedures for administering, transcribing, and recording medications; completes medication reconciliation following SSH policy and procedure.
- Documentation and plan of care: develops, discusses, and communicates a prioritized problem list/plan of care for each patient; documents outcomes and updates the plan of care as needed.
- Admission and ongoing assessment: initiates admission assessment within 8 hours of admission; identifies and documents patient/family teaching needs upon admission and throughout hospitalization; continues assessment and documents care needs within established nursing practice.
- Pain management and comfort: assesses and documents patient level of comfort using appropriate pain scales and tracks response to interventions (pharmacologic and non-pharmacologic) per hospital policy.
- Care coordination: facilitates discharge/transfer planning with a goal of 12:00pm discharge when possible; communicates barriers using the chain of command.
- Safety and quality: fosters a culture of safety; verifies patient identification with two identifiers prior to invasive procedures and during care, including time-out and proper documentation; adheres to hand hygiene guidelines and universal precautions; maintains regulatory-compliant environment.
- Regulatory and privacy: complies with legal, regulatory, and ethical standards; safeguards patient privacy and security of information; complies with mandatory education requirements for compliance, privacy and security programs.
- Professional development and teamwork: maintains own professional development with evidence-based practice; seeks feedback and collaborates with colleagues to promote a positive work environment.
- Technology and downtime: utilizes departmental software and technology as required; reviews emails and training resources; understands downtime procedures.
- Minimum Education – Preferred:
Graduate of an accredited School of Nursing. - Minimum
Work Experience:
Previous Acute Care RN experience preferred. - Required
Licenses/Certifications:
RN – Registered Nurse;
Basic Life Support (BLS) required; additional certifications such as ACLS, PALS, TNCC, TCAR, NRP, etc., may be required depending on department.
Shift details: 24 hours per week, 7:00 PM – 7:30 AM, rotating weekends and holidays.
