Residential Rehab Coordinator

Sun-Thurs (Sun, Mon, Tues, 9 a‑5 p & Thurs, Fri, Noon‑8 p)
Responsibilities- Advocate for client caseload
- Develop, implement, and monitor treatment plans
- Oversee medication management
- Fiscal management of funds
- Serve as primary liaison for service providers, family, and guardians
- Utilize an electronic health record system for client notes
- Perform a variety of administrative, communication, and reporting functions
- Enter notes into Avatar; review notes entered by other residential staff for clarity and accuracy
- Report incidents to Site Manager and other providers ensuring confidentiality
- HS diploma and at least 1 year of related field experience; additional education toward an AS or BA in health care is preferred.
- Excellent communication and writing skills
- Time‑management and ability to organize multiple priorities
- Calmness and de‑escalation of stressful situations
- Good judgment and problem‑solving ability
- Dependability and flexibility
- Team‑work orientation and role‑modeling for other staff
- Ability to form therapeutic relationships with challenging clients
- Computer proficiency and ability to use an electronic health record system
- Valid driver’s license, safe driving record, and access to a personal vehicle
Annual salary range: $22 – $22 USD
