Hourly; cases range 4–12 hours at any time in the day, evening or night.

Enables elderly & disabled individuals to achieve maximum independence, to participate in their own plan of care, and to live with dignity while receiving quality care in their own homes.

• Provide personal hygiene and grooming care to clients.
• Assist with toileting, ambulation, and related tasks.
• Provide meals to meet clients' nutritional needs.
• Report changes in clients' condition to nurse, supervisor, or client care coordinator.
• Perform routine housekeeping and home management duties.
• Record each client's care on the care plan and submit care plan in a timely manner.
• Provide emotional support for clients and their families.
• Assist with routine therapy exercises.
• Participate in case conferences as requested.
• Attend all required in-service education sessions.
• Use computer software programs and smart phone for required education.
• Represent the company in a positive and professional manner through a high level of customer service.
• Think critically without direct supervision and the ability to escalat when appropriate.

• Two years of continuous work experience.
• One year of previous patient care experience; specific experience as designated by work area may be required.
• New Grad CNA's are welcome.
• Must have a current and valid Driver’s License in good standing along with reliable transportation and proof of auto insurance.
