Shift Detail: Hourly cases range anywhere from 4-12 hours at any time in the day, evening or night.

Hartford Health Care Independence at Home enables elderly & disabled individuals to achieve maximum independence, to participate in their own plan of care, and to live with dignity while receiving quality care in their own homes. Our staff of highly trained professionals treat patients holistically, considering family and living situations for the most appropriate care plan.

Responsibilities:

• Provide personal hygiene and grooming care to clients.
• Assist with toileting, ambulation, etc.
• Provide meals for clients’ nutritional needs.
• Report changes in clients’ condition to nurse, supervisor, or client care coordinator.
• Perform routine housekeeping and home management duties.
• Record each client’s care on care plan and submit the care plan in a timely manner.
• Assist with providing emotional support for the clients and their families.
• Assist with routine therapy exercises.
• Participate in case conferences as requested.
• Attend all required in‑service education sessions.
• Use computer software program and smartphone for required education.
• Represent the company in a positive and professional manner through a high level of customer service.
• Think critically without direct supervision and ability to elevate when appropriate.

Qualifications
:

• Two years of continuous work experience.
• One year of previous patient care experience; specific experience as designated by work area may be required.
• New Grad CNA’s are welcome.
• Must have a current and valid Driver’s License in good standing along with reliable transportation and proof of auto insurance.
