Hartford Health Care Independence at Home enables elderly & disabled individuals to achieve maximum independence, to participate in their own plan of care, and to live with dignity while receiving quality care in their own homes. Our staff of highly trained professionals treat patients holistically, considering family and living situations for the most appropriate care plan.

• Provides personal hygiene and grooming care to clients.
• Assistance with toileting, ambulation, and other activities of daily living.
• Provides meals for clients' nutritional needs.
• Reports changes in clients' condition to nurse, supervisor, or client care coordinator.
• Performs routine housekeeping and home management duties.
• Records each client's care on care plan and submits care plan in a timely manner.
• Assists with providing emotional support for the clients and their families.
• Assists with routine therapy exercises.
• Participates in case conferences as requested; attends all required in-service education sessions.
• Has the ability to use computer software program and smartphone for required education.
• Represents the company in a positive and professional manner through a high level of customer service.
• Can think critically without direct supervision and can escalate when appropriate.

• Two years of continuous work experience.
• One year of previous patient care experience; specific experience as designated by work area may be required.
• New Grad CNA's are welcome.
• Must have a current and valid Driver’s License in good standing along with reliable transportation and proof of auto insurance.
