* We are seeking a dedicated Hospice Registered Nurse to provide compassionate home
• based care. This role involves working 36 hours per week, Friday through Sunday, from 8 PM to 8 AM.

* Key Responsibilities:

* • Deliver quality hospice care in patients' homes.

• Complete same
• day documentation using HCHB/OASIS.

• Manage a caseload of 20
• 25 cases weekly.

• Attend weekly in
• person office meetings.

• Ensure compliance with all care standards.

** Requirements:*
* • Active RN License and BLS certification.

• 2+ years of hospice experience.

• Proficiency in HCHB/OASIS, including Oasis

• Strong independent work and time management skills.

• Reside within 30 minutes of the assigned area.

• 3 days of orientation.

• Mileage reimbursement provided.

• call duties; flexible floating requirements.
