Position: Case Manager
Job : 866
Location: Westwood MA
Shift: Monday-Thursday 8am - 4pm, Friday 7:30am - 3:30pm
Responsibilities- Assists individuals served within the program obtain and maintain employment that is consistent with their employment goals.
- Collaborating with an exceptional team to plan and engage in the advocacy and support of our individuals, attending regular meetings as necessary.
- Supporting individual’s self-employment, education, social growth, health wellness, communication skills, community mobility, and volunteerism.
- Provide support through case management (electronic (Icentrix) and physical case records).
- Writing and implementing Individual Support Plans (ISP) and completing all documentation.
- Monthly and weekly progress notes for individual goals and objectives. Maintaining consistent communication with individuals and members of the ISP team.
- Provide on and off-site support for Life Enhancement Curriculum: assisting individuals served in areas of employment, social, creative, and health & wellness development and training.
- Leading individuals served in community volunteer and learning experiences. Daily completion of payroll and billing documents.
- Other duties and/or assignments at our different locations based on the needs of the team and the individuals you serve.
- Experience working with Individuals with /DD
- A Bachelor's degree or an associate degree in a related field with two years of experience, or HS diploma and five years of related work experience in Human Services.
- Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
- Valid Driver's License required
- Valid MAP certification is preferred
- Excellent verbal and written communication skills
- 403(B) matching up to 3%
