Job Title: Case Manager
Number of Openings: 1
Hours: 40 hours per week
Days: Monday – Thursday (8:00 – 4:00);
Friday (7:30 – 3:30)

- Create and maintain a sense of community among participants, families, staff and co-workers.

- Comply with all principles of Human Rights, D.P.P.C., Department of Developmental Services, the Division of Medical Assistance and all other applicable funding regulations as adhered to by Lifeworks.
- Provide individual and small group instruction in self-care, activities of daily living, communication, social and motor skills.
- Provide direct care for ADLs and group activities. Lift and transfer individuals as appropriate to their needs.
- Coordinate services with Interdisciplinary Team.
- Communicate respectfully and pro-actively with families, residential staff and all members of participants’ teams.
- Develop and implement Day Habilitation Service Plans (DHSP), including:
- Maintain progress notes and document progress on objectives
- Maintain assigned participant records so that all information is current and filed appropriately
- Write bi-annual and annual summaries
- Complete all required documentation in a timely and accurate manner
- Participate in scheduled supervision sessions with Assistant Director.
- Participate in program-wide staff meetings and case review meetings as scheduled.
- Participate in team meetings regarding general program development, including curriculum development and schedules.
- Demonstrate good computer skills and learn to utilize the agency’s computer programs for documentation of necessary data.
- Attend in-service trainings as required.
- Work in a team environment.
- Multitask and prioritize tasks.
- Strong written and verbal communication skills along with excellent interpersonal skills.
- Comply with all applicable State and Federal regulations, agency policies and procedures, and personnel practices.
- Protect the health and safety of all participants and staff.
- Perform other duties as requested by supervisor(s).
BA/BS in Human Services/related field or associate degree plus 3 years related experience with persons with /DD or Human Services, or HS Diploma/GED plus 5 years of related experience with persons with /DD or human services.
