About The Role
This is a full-time primarily remote role for a Behavioral Health Care Manager. The Behavioral Health Care Manager is a core member of Revere’s collaborative care team, providing care management and acute therapeutic interventions for high risk members; collaborating with community, state agency, and hospital care teams; and ensuring quality care delivery through member-centered care.
Role responsibilities include:
- Provide care management and acute therapeutic interventions for high risk members as part of a collaborative care team.
- Collaborate with community, state agency, and hospital care teams to ensure coordinated care.
- Ensure quality care delivery through member-centered approaches.
Required and preferred qualifications
- Experience working in a post-acute care environment strongly preferred.
- Knowledge of community supports and resources, and ability to learn and adapt to new programs and initiatives.
- Ability to assess for safety and implement safety/crisis planning.
- Ability to learn and use multiple medical record systems across platforms in a high paced and self-directed environment.
- Interpersonal and clinical skills to engage members and families in therapeutic relationships.
- Able to cope with time pressure while respecting the needs of the member and organization/department.
- Ability to solve problems with sound judgment and understanding of issues, even when dealing with emotional topics.
- Ability to perform a wide variety of care management tasks and shift focus as demands change, managing transitions effectively.
- At least 2 years of experience in behavioral health care management and/or acute behavioral health care focusing on outpatient/inpatient utilization, case management and discharge planning.
- At minimum, a bachelor’s degree in Social Work, Psychology, Counseling, Rehabilitation, or other relevant field.
- Licensed in one of the following: LICSW; LCSW; LMHC; LMFT; RN with behavioral background.
