Registered Nurse​/Hospice

Shift: Day 5x8-Hour (08:00 - 04:30)

Description:

This position will support hospice patients and associated caregivers at their residence (Home, Assisted Living or Skilled Nursing Facility). Our geography includes Baltimore County and Baltimore City. Patients assigned to the case manager will be in a sub-territory within the county/city to minimize windshield time. For reference: our main areas are Reisterstown, Randallstown, Owings Mills, Pikesville, Park Heights, Roland Park, Towson/Parkville, Gwynn Oak, Windsor Mill, Woodlawn.

This position will require travel to the patients.

Mileage reimbursed per IRS Standard

RN Case manager will:

Evaluate patient for continued hospice eligibility
Conduct head to toe assessments
Monitor and assess symptoms and work with the interdisciplinary team to maximize comfort
Determine goals for patient and family at end of life and assist in working towards and meeting goals to include involving the other disciplines as indicated
Educate and empower caregivers on end of life treatments and interventions
Prepare for case conference on each patient every 14 days and attend interdisciplinary team meeting weekly
Order necessary medical supplies and refills of medications
Create a plan of care for the hospice aide and supervise their work
Update the Plan of care for patient on an ongoing basis
