Our Full‑Time Patient Care Coordinator (PCC) will be part of our agile team, making the future of complex chronic care delivery the way it should be – human‑centered and personalized. The PCC is the heartbeat of the support team. In this key role you’ll showcase your passion for administrative, hospitality, customer service, and hands‑on clinical skills; all in support of patient care.
You will report to the Senior Director of Operations. This role will be on‑site and a current Medical Assistant certificate is preferred.
- Assist with vital collection and documentation, as well as other general clinical assistance.
- Deliver a positive, warm greeting to each patient as they walk in the clinic.
- Dialogue via phone, email, text (via platform), and inquiries from referring providers and patients in a timely and accurate manner.
- Receive packages, order supplies, restock supplies, perform light housekeeping, and general clinic support functions.
- Own suite and supply sanitation between patients.
- 2+ years of clinical experience.
- Active CPR certification (or willing to obtain certification within 90 days of hire).
- Familiarity with injection therapy (infusion therapy good, but not required).
- Strong understanding of clinical processes and protocols.
- Licensed Medical Assistant (MA) highly preferred.
- Ability to navigate and chart in Electronic Health Record (EHR) systems.
- Ability to analyze, order, and synchronize multiple high‑impact priorities in a fast‑paced work environment.
- A positive, upbeat team player.
- Strong customer service skills and an empathetic disposition.
- Optimizes work processes – aware of the most effective and efficient processes to get things done, and focused on continuous improvement.
