Make Every Moment Matter as a Hospice Aide (CNA)! Are you a caring and dedicated Certified Nursing Assistant looking to make a meaningful impact? Continuum Hospice is seeking a Full Time Hospice Aide (CNA) to join our compassionate team, providing hands‑on care that brings comfort and dignity to patients at the end of life. Working closely with our nursing team, you’ll ensure each patient’s physical, emotional, and personal needs are met with respect, empathy, and professionalism.

• Provide personalized care: follow the plan of care to ensure patient comfort and well‑being; attend promptly to patient and family requests with compassion.
• Assist with personal care: help with bathing, oral hygiene, changing bedding, clothing, and shampooing to maintain dignity.
• Deliver outstanding customer service, going above and beyond to ensure optimal patient satisfaction.
• Support families: share knowledge and expertise to guide them in caring for their terminally ill loved ones.
• Maintain accurate documentation to meet agency and regulatory standards.

• High School Diploma or G.E.D. required.
• Current, unencumbered CNA certificate in the state.
• Minimum one year experience as a CNA in an acute or long‑term care setting; hospice experience is a plus.
• Current CPR certification if state‑mandated.
• Valid driver’s license, vehicle insurance, and dependable transportation.

$21.00 – $22.00 per hour (average pay range). The pay range is a guideline; final compensation will be determined based on experience, qualifications, and responsibilities.
