• Conduct in-home, phone, or virtual assessments of seniors
• Determine eligibility for food assistance programs like Meals on Wheels
• Keep accurate records using our easy case software
• Follow up with clients regularly to ensure their needs are met
• Work with other local programs to provide holistic support
• Help us improve services by tracking outcomes and identifying gaps

Social Worker

• A degree in social work, public health, nutrition, or a related field (or equivalent experience)
• Experience in social services, outreach, or nutrition support (a plus!)
• Great communication and organizational skills
• A caring heart and culturally sensitive approach
• Basic computer skills and database experience
