Registered Nurse - Patient Aligned Care Team

The Patient Aligned Care Team (PACT) primary purpose of this position is to executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex primary care. This nurse provides gender-specific direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.

This position is located at the Lenore Community Based Outpatient Clinic in Williamson, WV.

The Registered Nurse performs initial history and assessment, patient teaching/education, Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) is responsible for providing competent, and verification of informed consent. They demonstrate clinical competence and leadership in delivering and improving holistic care through collaborative strategies with others such as patients, their family members/ significant other and members of the interdisciplinary team.

Major duties include, but are not limited to:

* Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex patient care.

* Demonstrates performance and leadership that is broad enough to improve the care for a group of patients.

* Responsible for the documented outcomes at the program or service level.

* Program is defined as a substantial coordinated group of activities impacting patients and/or employees.

* Leads and organizes delivery of care to ensure continuity of care and peer accountability for practice in alignment with the ANA Standards of Nursing Practice.

* Leads staff to identify, analyze and address ethical issues that impact patients, caregivers and staff beyond the immediate practice setting.

* Leads with emotional and social intelligence. Demonstrates resilience in coping with challenging and stressful situations, to assist others in increasing their self-determination and competence beyond the immediate practice setting.

* Initiates and leads interdisciplinary teams that utilize critical thinking to improve nursing practice beyond the immediate practice setting.

* Leads educational initiatives that expand knowledge to improve outcomes beyond the immediate practice setting.

* Accountable for personal and professional career goals with outcomes that have an impact beyond the immediate practice setting.

Pay:
Competitive salary, regular salary increases, potential for performance awards
