Registered Nurse - Patient Aligned Care Team

Overview

The Patient Aligned Care Team (PACT) primary purpose of this position is to provide a variety of nursing care and practice which do not require full professional nurse education, but require skills represented by the licensing of practice and vocational nurses.

This position is located at the Lenore Community Based Outpatient Clinic in Williamson, WV.

The Registered Nurse performs initial history and assessment, patient teaching/education, Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) is responsible for providing competent, and verification of informed consent. They demonstrate clinical competence and leadership in delivering and improving holistic care through collaborative strategies with others such as patients, their family members/ significant other and members of the interdisciplinary team.

Major duties include, but are not limited to:
