Rivers Health is seeking a full-time Nurse Practitioner for our Wound Care Center.

Job Responsibilities:

• Provide comprehensive diagnosis and wound assessment, managing individualized wound care treatment plans for healing or chronic maintenance.
• Coordinate care including nursing, nutrition, medications, dressings, and adjunctive therapies.
• Perform physical exams and preventive health measures within guidelines.
• Order, interpret, and evaluate diagnostic tests, collaborating with primary care providers for comprehensive care.
• Record findings and develop care plans and prognoses based on patient conditions.
• Discuss cases with providers and other health professionals to prepare comprehensive care plans.
• Submit healthcare plans and goals for periodic review and evaluation.
• Prescribe or recommend drugs and therapeutic procedures.
• Refer patients to specialists or for additional treatment resources.
• Demonstrate excellent customer service and foster teamwork.
• Participate in quality improvement activities and other duties as assigned.
