Rehab liaisons identify potential patients in hospitals or other healthcare settings who may benefit from inpatient rehabilitation. They assess patients' medical history, current condition, and therapy needs to determine if they meet the criteria for admission to a rehabilitation facility. They coordinate the referral-to-admission process for patients and educate the community and other stakeholders about the unit's services and programs.

Minimum Qualifications Education, Certification, and/or Licensure
  • Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
  • Valid driver's license.

  • The Rehab Liaison is responsible for cultivating referral relationships. This role is crucial for driving growth through patient referrals and involves developing strong relationships with various stakeholders, including patients, family members, physicians, and healthcare professionals.
  • The rehab liaison is expected to exercise sound judgment in handling the professional and confidential nature of healthcare sales, ensuring that you maintain the highest standards of integrity and professionalism in all interactions.
  • The position blends professional growth with community connection, emphasizing the importance of collaboration and communication in the rehabilitation process.
  • Educate community, referral sources, and physicians on unit programs and services.
  • Respond to and overcome admission barriers and follow up on admissions variables.
  • Conduct in-services and professional presentations to various groups about services and outcomes.
  • Clinical Liaisons perform clinical assessments of referred patients, prioritizing face-to-face connections.
  • Educate referred patients and families on acute rehabilitation programs.
  • Aid in streamlining the referral-to-admission process.
  • Liaisons partner with hospital case managers to create smooth patient transfer processes and to help remove payer-related and operational barriers to getting appropriate patients into the best next level of care.
  • Work with hospital case managers and physicians to identify and transition patients into the next level of care.
  • Develop and cultivate professional relationships with case managers, physicians, and clinicians in acute care hospitals.
  • Manage the referral process effectively to ensure timely admissions.
  • Accurately and efficiently complete the Pre-Admission Screen adhering to all policy, regulatory and payor guidelines.
  • Maintain knowledge of CMS and third-party payor inpatient rehabilitation requirements.
  • Demonstrate excellent customer service skills with focus on referrer satisfaction and account retention.
  • Develop strategies to grow market share in core business lines.
  • Develop business census based on strategic goals.
  • Utilize market analysis to identify new opportunities.
  • Creates quarterly goals and objectives and submits to supervisor.
  • Works with Director of unit and medical Director to identify opportunities for expansion of strategic business initiatives.
Physical Requirements
  • The National Institute for Occupational Safety and Health recommends minimizing the need for employees to manually lift patients and limit safe patient handling to 35 pounds. All transfers, lifts and re-positioning will be completed utilizing the lift equipment and/or other patient handling aids as indicated by the patient's profile and appropriate algorithms.
  • Heavy/Hard work:
    Work requires strength and/or stamina, lifting, moving, stooping, reaching, standing, walking, and carrying of materials and equipment weighing 40+lbs.
  • Visual acuity must be within normal range. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
  • Hearing within normal range is required.
