Responsibilities- Enrich lives as a nursing center Social Services Specialist, collaborating with patients/residents and their families/significant others to identify strengths and social, emotional, and mental health needs and obtain services to meet those needs.
- Provide patients/residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life, in accordance with the NASW Code of Ethics and compliance with federal, state, and local guidelines, regulations, company policies and procedures, and standards of care.
- Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being; educate staff on the role of social services.
- Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
- Hold meetings with patients/residents, families, and significant others as requested to provide support and information to improve quality of life.
- Perform discharge planning activities such as family/patient education, nursing center documentation, and make referrals to appropriate agencies and suppliers.
- Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit the patient/resident population.
- Bachelor's degree in social work or human services field required.
- Must possess any certifications/licensures as required by the state of employment to practice in long-term care.
