MDS Coordinator (RN) – 10K Sign on Bonus

Position Overview

The MDS Coordinator supervises and coordinates all MDS and care plans for residents, ensuring compliance with state and federal regulations in a timely manner.

Key Responsibilities
  • Conduct and coordinate the MDS and Care Plan according to facility policies and procedures.
  • Inform all care team members of care plan due dates and ensure completion.
  • Develop and revise resident care plans quarterly or with significant changes.
  • Ensure all PPS and state Minimum Data Set assessments are completed correctly.
  • Coordinate care plan meetings and encourage team participation.
  • Complete and submit MDS to state/federal agencies per Medicare/Medicaid guidelines; maintain the Resident Assessment and Care Plan Schedule.
  • Ensure resident rights are observed during the MDS process, including confidentiality and privacy.
  • Maintain effective communication among departments so MDS and care plans remain current.
  • Monitor resident care during the MDS process and report concerns to the Executive Director and Director of Nursing Services.
  • Assist with pre‑admission screening to estimate potential resident RUGs group.
Qualifications
  • Current RN (or LPN) nursing license in the state of employment, active and in good standing.
  • Experience in Long Term Care preferred.
  • Computer skills including data entry and word processing; knowledge of MDS 3.0.
  • Knowledge of nursing and medical practices, laws, regulations, and guidelines for skilled nursing facilities.
  • Ability to plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures for quality care.
  • Active CPR license or the ability to obtain within the first 30 days of employment; must be kept active throughout employment.
